As a member of the U.S. team at the 2007 Parapan American Games, Jacob Counts had his best performance of the tournament in the gold medal game against Canada. He came off the bench to score 11 points and grab 10 rebounds in a 52-50 victory. In Beijing, however, Counts and Team USA were unable to defeat Canada in the semifinals.

Counts started playing wheelchair basketball in 1999 with a local club team, the Cincinnati Slammers and heard about the Paralympic Games through a veteran Wheelchair Basketball player while training as a member of the University of Wisconsin-Whitewater (UWW) Warhawks Wheelchair Basketball Team.

Counts graduated from UWW in 2006 with majors in human performance and recreation. Currently, he is playing wheelchair basketball professionally in Europe.

In addition to wheelchair basketball, Counts enjoys whitewater and flat water kayaking.
